We are happy to announce that our website has been updated to give our clients a more user-friendly experience. While we hope you will take the time to check out the redesign, there are a few pages we wanted to bring to your attention.

This page has all of the forms you may need. Here you can find our newly revised claim form, Direct Deposit forms, a list of permitted expenses, and much more. You can also find the FSA Ready Receipt form, for instances when your service provider is unable to provide you with a receipt or documentation, which are required for reimbursement. Simply print out that form, have the service provider complete it, and then submit that to us along with a completed claim form, and we will be able to use that to process your claim.

Our claims department is staffed by individuals whose primary job is to make sure that your claim is processed both swiftly and accurately. There are a number of things you can do to help us do a better job in the processing of your claim. While we always do our best to process each claim to the best of our ability, however following the recommendations on this page will help to ensure that your claim is able to be processed quickly and that the reimbursement you expect, is the reimbursement you receive.

Our Video page provides answers to some of our most common questions. If you will be submitting any claims, you will want to take the 4 minutes to watch our first video, How to fill out the claim form. While we tried to design the form so that it is easy and intuitive to complete, this video will cover a number of tips that will make the process of completing the form much easier.

Additionally, you can find a pair of videos showing how you can access your online account. We recommend taking a few minutes to review these videos, as they will enable you to access your account 24/7. This is incredibly helpful, as you will not need to wait until our office is open to access your account. If you have never logged in to access your account, you will want to watch the First Time Login Instructions video. Once you have logged in to the system, you will want to watch our How To Use The Web Portal video, as this will show you how to access your account information. It will provide info such as how much funds are available, whether or not we have received your claim, what claims have already paid out, and which are still pending.

Finally, if your employer has provided you with a MySource Debit Card, we recommend watching the final video on that page, How To Use The MySource Debit Card. This video shows you can to determine which of your card swipes have resolved, and which require substantiating documentation to be sent in. If your card is declined, you are able to run a card decline history to see why the card was declined. If your card is lost or stolen, you also have the ability to order a replacement card.

Keep an eye on this page, as we will be adding more videos in the near future. Diversified Administration is working to bring our clients a mobile app to access your account on both the iPhone and smart phones running the Android operating system. As soon as this becomes available, the information will be posted on this page.
